Description:

Function as a community health worker conducting no-medical drivers of health screenings, conducting home visits and delivering ongoing case management to connect participants with needed community and social services. Must have a valid Texas Driver’s license and comply with AP 2-2 policy. Must be a certified community health worker or medical assistant with strong data entry skills using electronic health records, client record documentation, Microsoft intermediate skills, active listening and motivational interviewing skills.

SPECIFICATIONS           
It is mandatory that all Houston Health Department Temporary Employees are REQUIRED to be current on their vaccinations. Upon your initial visit to Employee Health, expect to receive a Tuberculosis Skin Test, administration of vaccines that are not up to date, and blood work if needed.

Necessary needs before seeing Houston Health Department Nurse:
BRING IMMUNIZATION RECORDS (Shot records)
Eat and Hydrate before reporting to Employee Health for blood 

MINIMUM QUALIFICATIONS   

KNOWLEDGE:  Requires a Bachelor’s degree in Education, Behavioral Science or a closely related field such as Public Health

EXPERIENCE: No experience is required. Directly related professional experience in Public Health, Education or a closely related field may be substituted for the education requirement on a year-for-year basis.

PHYSICAL EFFORT: The position occasionally requires stooping or bending. Occasional very light lifting, such as three or four reams of papers or books (up to 20 pounds or an equivalent weight) may be required

WORK ENVIRONMENT: There are no major sources of discomfort, i.e., essentially normal office environment with acceptable lighting, temperature and air conditions.

PHYSICAL SKILL: Requires the ability to make simple gross motor responses within large tolerances.
